01Who we are

Community Wellness Group is a private, independent company that guides people aged 55 and over and their families in Los Angeles and San Bernardino Counties, California, and connects them with service providers and centers.

We are not a government agency, an insurance company or a medical center.

02What data we collect

We only collect the data you give us voluntarily when you message us on WhatsApp, call us or complete the form on this site: name, phone, email, city or ZIP code, age range of the person who needs help, preferred language, service of interest, preferred contact time and any detail you choose to include in your message.

We also record basic technical browsing data, such as device type and pages visited, to understand how the site is used.

03What we do not ask you for

We do not ask for your social security number. We do not ask about your immigration status or your family status. We do not ask for detailed medical diagnoses, clinical histories or medication lists at first contact.

If someone claims to represent Community Wellness Group and requests any of this, do not provide it and contact us.

04What we use your data for

To contact you and answer your inquiry. To understand which service might fit your situation. To refer your case, with your authorization, to the corresponding provider or center. To follow up and confirm whether you were taken care of. And to improve the quality of our guidance.

We do not use your data to make decisions about your eligibility or your coverage, because those decisions are not ours to make.

05Who we share it with

We share your data with the provider or center we refer your case to, and only with your prior authorization. We share it with technology service providers that allow us to operate, such as the form delivery service and our contact record tools.

We do not sell your data. We do not hand it to third parties for advertising purposes.

06Calls and messages

By checking the authorization box on the form, or by messaging us on WhatsApp, you agree that we may contact you by phone, text message or WhatsApp regarding your inquiry.

You can ask us to stop contacting you at any time, by replying to any of our messages or writing to our email address.

07How long we keep it

We keep your data for as long as it is necessary to follow up on your case and to meet our legal obligations. After that period we delete it or anonymize it.

08Your rights

You can ask us to tell you what data of yours we hold, to correct it if it is wrong, and to delete it. You can also withdraw your authorization for us to contact you.

If you reside in California, state consumer protection law may grant you additional rights over your personal data.

To exercise any of these rights, write to us and we will respond.

09Cookies and measurement

This site may use measurement tools to know how many people visit it and which pages they read. That information is aggregated and we do not use it to identify you personally.

10Changes to this policy
