What it is

Home Health means clinical services delivered directly at home, instead of the person having to travel to a facility.

It requires a physician order and depends on each person eligibility and coverage. It is not a service you hire on your own the way you would hire household help.

  • Skilled nursing
  • Physical Therapy
  • Occupational Therapy
  • Speech Therapy
  • Certain Home Health Aide services

How it differs from Home Care

  • Home Care focuses on personal assistance and daily activities
  • Company and companionship
  • Meal preparation
  • Help with bathing and dressing
  • Other activities of daily living

Who it is usually considered for

  • People discharged from the hospital who need clinical follow up at home.
  • People who have difficulty traveling to therapy frequently.
  • People who need wound care, medication management or rehabilitation.

What is worth knowing

  • It requires a physician order. Requesting it is not enough.
  • It is not the same as Home Care: they are different services, with different requirements and coverage.
  • The duration and frequency of visits are set by the care plan, not by the family.

Eligibility and final coverage are determined by the plan, the program or the authorized provider, never by Community Wellness Group.
