Community Wellness Group does not directly provide all of these services. We explain which ones exist, which ones might apply to your case, and we connect you with the center or provider that fits.
Five services, explained without jargon
Each one has different requirements and depends on the program and the coverage each person has. Open the one you are interested in and, if you have questions, write to us and we will talk.
Adult day center
They spend the day with company, meals and activities, and come home to sleep in the afternoon. It is not a nursing home.

One important clarification
None of these services starts simply because you request it. Eligibility and final coverage are determined by the plan, the program or the authorized provider. What we do is explain the landscape, make a preliminary assessment and connect you with the right people.
Not sure which one your family needs
That is completely normal. Write to us, tell us the situation, and we will tell you what options exist for your case. And also which ones do not apply.
