Community Wellness Group is an intermediary and service coordinator. We guide people aged 55 and over, identify their needs and connect them with the provider or center that fits.

Many older people, especially those who are retired, do not know they may have access to certain benefits and services. In many cases their families do not know either.

Community Wellness Group wants to be the bridge between the older person, their family and the services they need.

Four things we do not compromise on

We speak plainly

If we cannot help you, we say so right away. We would rather lose a contact than waste your time.

We do not promise what is not ours to promise

We do not say you qualify or that you are approved. The program or the provider decides that, and we always explain it that way.

In your language, without rushing

A real person answers, not a robot. And the conversation lasts as long as it needs to last.

No pressure

There are no spots running out and no offers expiring. We are here whenever you want.

Behind every conversation there are people with experience in Southern California health services and a provider network built case by case.
